The Early Assessment and Support Alliance (EASA) was created to address the needs of young people ages 12-25 who are experiencing their first episode of psychosis. The purpose of the program is to provide intensive supportive services to reduce symptoms and mitigate the impact of psychosis on the individual family and society. EASA includes a Fidelity program and an EASA Step-Down program. The Step-Down program expands current services for clients who have completed the Fidelity program providing ongoing treatment and skills building. Both EASA Fidelity and Step-Down services may include psychiatric medication management nursing support occupational therapy individual family and group therapy case management vocational/educational support psycho-education peer support and crisis intervention. The EASA Nurse provides comprehensive nursing services to EASA participants using professional nursing diagnosis and judgment in assessment evaluation screening education medication services promoting overall health and referral.

This half-time position has a three-day-per-week schedule (Monday/Wednesday full day Thursday afternoons) and includes time in the community for client/family outreach or agency education/outreach.

Teleworking/Hybrid: At this time this position is designated to hybrid meaning you may be working remotely (from home) some of the time and you will be required to attend meetings and report in person regularly. The designation of Hybrid Telework may be subject to change at a future time. Currently all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

As the Community Health Nurse - FTE .50 duties include:

Assessment Treatment and Psychoeducation

  • Engage and build rapport with individuals and families referred to EASA Fidelity and Step-Down services who may be experiencing active psychosis paranoia and delusions.

  • Perform comprehensive health assessments of clients by completing a health history to identify actual and potential health needs based on physical mental/emotional financial and social factors.

  • Work with EASA psychiatrists and clinical team members to participate in assessments provide liaison to the primary care providers office identify medical needs and follow up on diagnostic testing.

  • Provide illness education to individuals and families.

  • Help monitor general health symptoms and side effects.

  • Conduct risk assessments and crisis intervention when needed.

  • Administer medication including injectable medication.

  • Services are offered in the community home or clinic.

  • Document all assessments service notes outcome measures and local state or federal measures or required forms.

Team-Based Care

  • Participate in clinical and administrative meetings retreats and individual case staffings.

  • Communicate with assigned Mental Health Consultant and other team members to coordinate nursing services with other treatment goals and activities.

  • Engage with families caregivers and/or community as part of the multidisciplinary team plan.

  • Facilitate psychoeducation groups or other groups and participate in program events such as workshops retreats etc.

  • Attend Direct Clinical Services (DCS) Behavioral Health Division (BHD) statewide EASA meetings and training as requested or required based on County policies EASA program fidelity guidelines and/or professional licensure/certification.

Community Outreach

  • Assist with outreach to schools colleges emergency rooms community services providers crisis lines etc. to provide program information referral information and general education about psychotic disorders and first breaks in young people.

Required Minimum Qualifications/ Transferable Skills*:

  • Valid license to practice as a registered nurse issued by the State of Oregon; as outlined by Oregon Revised Statutes (ORS) 678.021

  • One of the following:

    • A Bachelors degree in nursing from an accredited nursing program; OR

    • An Associates degree in nursing from an accredited nursing program AND two (2) years of registered nurse level experience is required.

  • BLS/CPR certification required

Other:

  • Must have a valid Drivers License.

  • This position requires a Background Investigation which may include being fingerprinted.

Preferred Qualifications/Transferable Skills:

  • Experience providing nursing services within a mental health system

  • Understand cultural/racial/ethnic factors including systemic advantage or disadvantage acculturation intergenerational immigrant family challenges effects of trauma during immigration war separation from family/primary supports detention refugee camps: generational trauma and any factors related to a non-dominant culture affecting mental health and treatment.

  • Advanced knowledge of issues young children and families witnessing or being impacted by violence aggressive behavior depression anger problems behavior problems attention problems dealing with grief etc

  • Respect and appreciation for cultural and ethnic diversity.

  • Ability to apply an equity intersectional and anti-racist lens.

  • Ability to provide information/referrals to community mental health providers.

  • Crisis intervention including risk assessment of clients who may be involved in potentially life threatening situations.

  • Advanced skills in managing sensitive information and ability to support the community.

  • Knowledge of cultures customs and social experiences of communities experiencing marginalization and barriers to opportunity.

  • Ability to exercise good judgment diplomacy and tact in dealing with sensitive complex confidential information and situations in addition to acting professionally and empathically with individuals internal/external partners and communities.
