Were looking for a dedicated Health and Wellbeing Worker to deliver early interventions that support young people aged 821 (up to 25 with SEND).

Youll promote positive health and wellbeing through one-to-one support group activities and community partnerships.

Whats the role

The role involves holding a caseload of one-to-one support group work and delivering activities that empower young people to make informed life choices.
Youll work closely with health practitioners and community partners to provide holistic youth-centred support.

Using frameworks like Young-Thrive and Outcome Star youll measure impact and help shape services that truly meet young peoples needs.
You will work closer with practitioners within Lewishams GP Youth Clinics to provide a youth-led support for children and young peoples health and wellbeing.

The Health and Wellbeing Worker plays a key role in supporting young people to lead happier healthier lives.
Youll be someone who can build trusting relationships and provide guidance to help them overcome challenges and make positive choices.

Working as part of a dynamic multi-agency team youll connect young people with the right support and opportunities.

Your work will focus on promoting mental physical and emotional wellbeing through creative youth-led approaches.

Youll help shape programmes that respond to local needs and empower young people to thrive.

Location

The job is primarily based in Youth sites and buildings across Lewisham; but we operate a hybrid approach where you will be required to work both in the office and from home on a regular basis.

Special conditions

• Relevant professional qualification in Youth Work is needed e.g. Level 3 certificate in Youth Work practice and/or Level 3 Diploma
• Weekend working may be required for this job
• The job is subject to an enhanced DBS check

40404 - 41637

N.B: All appointments will be offered at the minimum spinal column point (SCP) for the grade of the role. The only exception to this will be when matching current salary (up to the maximum of the scale) which is subject to evidential proof.
