Leominster Public Schools is looking for a long term substitute Licensed Practical Nurse to cover for an extended leave.

Position Description:

Leominster Public Schools is looking for a licensed practical nurse to provide direct care to students.

Responsibilities include but are not limited to:

oEvaluate student health and report findings/changes to the RN parent/guardian teacher and physician when appropriate.

oDocument health office visits in the students electronic medical record.

oWork with the RN to address student health issues and concerns

oMaintain school health records according to state laws and school policy via paper files and the computer

oParticipate in yearly mandatory student health screenings

oAdminister medications and treatments to students as per MD orders DPH guidelines and school policy.

oParticipate in staff meetings and professional development opportunities if asked

Qualifications:

Graduate of an accredited LPN program

Current Massachusetts Nursing License

CPR certification

First Aid certification

Preferred: Three years nursing experience
