PRACTICE SPECIFICS:

The GI Hybrid Nurse Practitioner or Physician Assistant is a skilled healthcare professional responsible for delivering patient-centered care to individuals with gastrointestinal (GI) conditions in both inpatient (hospital) and outpatient (clinic) settings. This role utilizes advanced health assessment knowledge and decision-making skills while working both independently and in collaboration with physicians and other health care professionals. The NP/PA will provide health care to individuals and families emphasizing health promotion and disease prevention caring for patients ranging in age. Position requires high level of customer service skills to establish and enhance positive relationships with patients co-workers and others.

This position offers:

60% clinic

o Provide care for patients with chronic GI conditions.

o Educate patients on disease management medications and lifestyle modifications.

o Document patient encounters assessments and treatment plans in the electronic medical record (EMR).

40% hospital:

o Conduct initial assessments physical exams and diagnostic evaluations for hospitalist GI patients.

o Develop and implement treatment plans in collaboration with physicians.

o Monitor and manage acute GI conditions and provide pre- and post-procedure care.

o Coordinate care transitions between hospital and clinic settings.

Anticipated Schedule:

Monday Friday (Day Shift)

Clinic 36 patient contact hours/week

Hospital 40 patient contact hours/week

Rotating Weekend Coverage

EDUCATION:

1. Masters or Doctorate degree in Nursing or Bachelors or Masters degree in Physician Assistant Studies from an accredited program.

LICENSURE/CERTIFICATIONS REQUIRED:

1. Current licensure as a registered nurse in Minnesota and Wisconsin.

2. Current licensure as a Nurse Practitioner or Physician Assistant in Minnesota & Wisconsin (WI would need to be obtained within 3 months of hire).

3. Current certification in CPR.

4. National certification in applicable area.

5. Successful completion of Essentia Health credentialing process prior to practice.
