About the role

This varied and challenging position is based within the Riverglen Older Persons Mental Health inpatient unit and requires a broad range of skills interventions and advocacy from an energetic and flexible clinician. This role contributes within a skilled multi-disciplinary team to ensure a collaborative patient-centred recovery focused service to this vulnerable and complex population. The role of the Social Worker is to ensure a high standard of Social Work service is provided to our inpatients as well as their carers and support people. We are seeking an experienced skilled and flexible Social Worker with a passion for mental health older people and making a difference to join our team.

This Level 1 / 2 Social Worker permanent full-time or part-time (4 days/week) role is currently located at Greenwich Hospital but will move to Macquarie Hospital in North Ryde from March 2026 for a 3 year period.

HammondCare is a NSW Health affiliated organisation all NSW Health transferrable entitlements will be recognised.


The essence of this role

  • Provision of the highest standards of effective efficient and quality social work service to patients including appropriate support and advice to carers and families.

  • Participate as a member of the multi-disciplinary team in assessing patients needs setting joint care and recovery goals with the patients/carers and providing appropriate interventions.

  • Participate as a member of the HammondCare and Social Work teams in the application of the organisations mission and values to the pursuit of its objectives in a client-centred continuous quality improvement work environment.

Selection Criteria

  • A tertiary qualification in Social Work providing eligibility and membership to the Australian Association of Social Workers (AASW).Qualifications obtained overseas must provide AASW Overseas Assessment number.

  • Minimum 3 years post-graduate Social Work experience preferably within mental health or complex health context.

  • Practical knowledge and high level clinical skills in delivering a broad range of Social Work services and interventions for complex clients preferably including older persons mental health.

  • Demonstrated interpersonal consultation and reasoning skills necessary to ensure high clinical practice standards and contribute to strong team practice using a variety of media - oral computer and written.

  • Demonstrated skills and experience in interacting constructively and collaboratively with the internal multidisciplinary team and external service providers.

  • Experience in quality improvement initiatives aimed at enhancing service delivery including planning development implementation evaluation and professional education and training.

  • A current Drivers License and willingness to travel between HammondCares sites and services.
