EXPERIENCED Physician Assistant OR ANCC Accredited Nurse Practitioner to work a remote position managing in basket burden for on-site primary care providers 3-5 years clinical experience in Primary Care is required.
AANC Accredited Family Nurse Practitioners do not require direct clinic hours to certify and are welcome to apply.
Remote workers are not eligible to work remotely from the following states: California and New York.
Participate in call for second level triage Monday through Friday 8-5 pm
Primarily remote position with occasional in person meetings
Attend initial in-person onboarding
Reviews patient records messages results and history to assess their current health status and develop proper treatment plans
Identifies real or potential health problems and expected outcomes in response to management plan
Reviews patient data with Physicians & APPs for proper treatment
Collaboration with the care team to provide guidance and outreach to patients and their caregivers
Outreach to providers and staff for follow-up
Documentation of patient records and EHR
Complete billable services such as but not limited to: E-Visits
Participate in process improvement initiatives for the Inboxology program
Other duties as assigned

Qualifications:
Nurse Practitioner:
Required Masters Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a masters post-masters or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
3 years of relevant experience current relevant clinical experience
Previous experience functioning in a collaborative role as a Nurse Practitioner
