Job Summary
We are seeking a compassionate and detail-oriented Nurse Case Manager to join our dedicated healthcare this pivotal role you will be responsible for coordinating comprehensive care plans for patients ensuring they receive the medical services and support they need for optimal health outcomes. You will also perform daily reviews of Inpatient/Observation stays to assure care meets current standards that level of care is appropriate and apply federal Insurance Guidelines as applicable. Your expertise in patient advocacy interdisciplinary collaboration and resource management will be key to delivering exceptional care and guidance to our patients and their families. Job Requirements
- Current New York Registered Nurse (RN) license
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of 2 years of clinical nursing experience
- Previous Case Management/Utilization Review experience preferred
- Proficient with healthcare management and clinical software systems
- Familiarity with healthcare regulations and accreditation standards
- Excellent communication and interpersonal skills
- Ability to collaborate effectively with multidisciplinary healthcare team
- Strong organizational and time management skills
- Experience in discharge planning and patient education
- Ability to conduct and interpret patient assessments
- Knowledge of insurance coverage claims and reimbursements
- Ability to maintain confidentiality and handle sensitive information with discretion
- Ability to work independently with minimal supervision
- BLScertification or ability to obtain within a specified time frame
- Hazards include risk of exposure to communicable diseases blood and body fluids and exposure to computer equipment.
- Sitting or Standing for long periods of time; walking lifting sitting and bending.
