Job Summary
The Perinatal Social Worker (PSW) provides direct patient care psychosocial services to DAPHealth perinatal patients as part of the Comprehensive Perinatal Services Program (CPSP).The goal is to help patients understand and manage the biological environmental emotionaland social stresses of pregnancy to improve the health outcomes for themselves and their PSW will maintain in contact with patients from the start of their prenatal care through thepostpartum period. The PSW will conduct initial and ongoing psychosocial assessments;individual care plan development; psychosocial interventions/follow-ups; and referrals to thecommunity. The PSW is part of the interdisciplinary care team that includes the clinician nursemedical assistant registered dietitian and Comprehensive Perinatal Health Worker.

Essential Duties/Responsibilities

  • Provide psychosocial evaluation under the CPSP model on a short-term model
  • Assess patients strengths and needs in the context of their environment
  • Complete risk status assessments and comprehensive social work care plans forperinatal patients
  • Interview perinatal patients with psychosocial risk factors and assess for follow upservices and referrals each trimester or more frequently as indicated
  • Utilize appropriate organizational electronic communication systems and communicateby other appropriate means on patient plan of care
  • Monitor patients with high-risk psychosocial status with their primary OB clinician
  • Facilitate warm hand offs from OB clinicians to initiate CPSP psychosocial services
  • Provide consultation to clinicians and staff about individual patients psychosocial needs
  • Serve as a communication liaison between OBGYN and Behavioral Health department ifadditional services are indicated
  • Collaborate with other pertinent departments and community partners to providereferrals to services/programs throughout patients perinatal care
  • Document all services provided accurately and in a timely manner per establishedworkflows and in compliance with regulatory requirements and accreditation standards
  • Participate in special projects/programs/services related to CPSP such group education(baby showers) pop-up events etc.
  • Ensuring appropriate billing of psychosocial services as outlined in the CPSP model ofcare
  • Assess and improve CPSP social work quality and outcomes
  • Maintain program protocols for the delivery of psychosocial support services incoordination with CPSP Lead
  • Update psychosocial resources and referral procedure for local community agencies incoordination with CPSP Lead
  • Select and/or produce and maintain culturally sensitive psychosocial educationalmaterials
  • Provide ongoing training support and consultation to other CPSP practitioners andclinicians within the organization
  • Provide psychosocial orientation training for new CPHWs
  • Assist patients with crisis intervention community resources transportation needs orany psychosocial problem affecting their care
  • Utilize established reporting tools to manage case load and utilization of services.
  • Work toward meeting program performance indicators
  • Attend external meetings as needed to ensure the coordination of care and communityengagement as directed
  • Participate in an attend all meetings such as team program meetings CPSP specifictrainings clinic/departmental meetings huddles and trainings
  • Other duties as assigned

Qualifications

Education and Experience

  • Masters degree in social work or social welfare from a college or university with a SocialWork Degree program accredited by the Council on Social Work Education
  • One year experience in the field of maternal and child health
  • Understanding of the overall delivery of perinatal services with an in-depth knowledge ofthe psychosocial component of comprehensive perinatal care preferred
  • Experience working in a community health setting preferred
  • Bilingual (English and Spanish/Arabic) preferred
  • Current BLS certification obtained through the American Heart Association or AmericanRed Cross

Working Conditions/Physical Requirements

  • This position is on-site at a DAP Health clinic location with the possibility of hybrid
  • Able to lift/move up to 20 pounds move from place to place and stand for long periodsof time
  • Requires current and valid drivers license and current personal auto insurance
  • Able to travel to DAP Health locations throughout San Diego and the Coachella Valley
