Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Nurse Practitioner to provide and oversee psychiatric care for adults with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system. This position serves Wake and Durham counties with the office based in Raleigh. The Nurse Practitioner works as part of a multidisciplinary team to deliver person-centered recovery-focused treatment that promotes stability safety and successful community reintegration. This role offers an opportunity to make a meaningful impact by improving access to quality behavioral health care for a justice-involved population.

As a FACT Nurse Practitioner you play a vital role in transforming lives and strengthening communities. You bring hope and stability to individuals navigating both behavioral health challenges and involvement in the justice system. Through compassionate evidence-based care including medication management crisis intervention and collaboration with courts and community partners you help clients move toward recovery independence and reintegration. Your clinical expertise ensures that every person served by the FACT team receives the comprehensive coordinated care they deserve.

Position Highlights:

  • Provide comprehensive psychiatric care including evaluation diagnosis medication management and documentation for justice-involved individuals.

  • Conduct forensic psychiatric assessments to determine competency risk and treatment needs within legal and clinical guidelines.

  • Collaborate with courts correctional facilities and community partners to advocate for treatment-focused alternatives to incarceration.

  • Offer clinical consultation and supervision to the FACT team on psychopharmacology co-occurring disorders and forensic complexities.

  • Participate in multidisciplinary team meetings and reentry planning to ensure continuity of psychiatric care and coordination across legal and behavioral health systems.

Location: Wake and Durham counties with the office based in Raleigh

Compensation: Competitive rate of $140 per hour

Qualifications

  • Education:
    • Masters degree
  • Certification / Licensure Requirements:
    • Licensed Nurse Practitioner
    • Certifications: Crisis Prevention Intervention (CPI) certification required within 90 days of hire. CPR/First Aid certification required.
    • Valid Drivers License and Reliable Transportation required.
  • Experience / Knowledge Requirements:
    • Strong knowledge of criminal justice processes risk assessment and competency evaluations.
    • Familiarity with evidence-based practices for treating justice-involved individuals with mental illness and co-occurring disorders.
