Job Summary
The LVN Case Manager at DAP Health is responsible for coordinating patient care advocatingfor patients and managing case management services for the Non-RWP Non-EIS and Non-ECM populations. Working under the Director of Care Coordination or Nurse Manager this rolesupports providers by ensuring timely follow-ups accurate documentation and seamlesscommunication across healthcare teams. The goal of this role is to enhance patient outcomesreduce provider workload and improve workflow efficiency in a fast-paced clinical setting.
Supervisory Responsibilities: None
Essential Duties/Responsibilities
Care Coordination & Patient Advocacy
- Provide comprehensive case management services within the scope of an LVN
- Serve as a liaison between patients providers and healthcare teams to ensureseamless care transitions
- Develop and monitor individualized care plans in collaboration with providers
- Coordinate follow-ups referrals and prior authorizations as required
- Conduct patient outreach calls to track progress follow up on care plans and addressservice needs
- Provider & Clinical Team Support
- Assist providers by managing non-complex and complex cases reducing administrativeburdens
- Support medication adherence monitoring appointment scheduling and patienteducation initiatives
- Work closely with interdisciplinary teams to streamline workflow and improve patienttracking
- Provide coverage for other Care Advocates as needed
Administrative & Compliance Duties
- Ensure accurate and timely documentation of all case management activities
- Maintain compliance with HIPAA regulations and DAP Health policies
- Report urgent case management needs to the Director of Case Management or NurseManager
- Participate in team meetings case conferences and quality improvement initiatives
- Other duties as assigned

- Provide compassionate high-quality customer service to patients and their families
- Engage in ongoing training to enhance patient interactions and satisfaction
Safety & Workplace Compliance
- Adhere to workplace safety protocols and immediately report unsafe conditions
- Maintain strict privacy and security measures regarding patient and employeeinformation
- Other duties as assigned.
Qualifications
Required Skills/Abilities
- Strong communication and interpersonal skills to collaborate with patients and providers
- Excellent organizational and time management abilities
- Ability to multitask and work independently in a fast-paced clinical environment
- Proficiency in electronic health record (EHR) systems
- Bilingual Spanish/English preferred
Education and Experience
- Licensed Vocational Nurse (LVN) Active California License
- Current BLS certification obtained through the American Heart Association or AmericanRed Cross
- Two to four years of nursing experience preferred
- Experience in case management or care coordination
- Prior experience managing Non-RWP Non-EIS and Non-ECM populations
Working Conditions/Physical Requirements
- This position is on-site at a DAP Health location
- This job operates in an office setting and requires frequent times of sitting standingrepetitive motion and talking
- Ability to lift up to 24 pounds
- Workweek hours: Monday Friday 8 am to 5 pm
- Must be able to lift/move up to 40 pounds and stand for long periods
