Position Summary:
The Intake & Referral Specialist plays a critical role in the onboarding of new clients and coordination of referrals. This position requires strong communication skills a deep understanding of home health services and the ability to work collaboratively with healthcare providers patients and internal teams.
Key Responsibilities:
- Receive and process referrals from physicians hospitals and other healthcare providers.
- Conduct initial intake assessments to determine eligibility and service needs.
- Coordinate with clinical staff to ensure timely initiation of care.
- Maintain accurate documentation and records in compliance with regulatory standards.
- Communicate effectively with clients and families to explain services and answer questions.
- Collaborate with insurance providers and verify coverage.
- Ensure smooth transitions from referral to active service.
Requirements
Qualifications:
- Minimum 1-3 years of experience in a home health or healthcare intake/referral role.
- Some understanding of Medicare Medicaid and private insurance processes.
- Excellent organizational and multitasking skills.
- Proficient in Microsoft Office and electronic health record (EHR) systems.
- Bilingual in Spanish is preferred but not mandatory.
- High school diploma required; associate or bachelors degree in healthcare administration or related field preferred.
