We are a compassionate and client-focused home health organization dedicated to providing high-quality care to individuals in need. Our team is growing and we are seeking a detail-oriented and empathetic Intake & Referral Specialist to join us in supporting our mission.

Position Summary:
The Intake & Referral Specialist plays a critical role in the onboarding of new clients and coordination of referrals. This position requires strong communication skills a deep understanding of home health services and the ability to work collaboratively with healthcare providers patients and internal teams.

Key Responsibilities:

  • Receive and process referrals from physicians hospitals and other healthcare providers.
  • Conduct initial intake assessments to determine eligibility and service needs.
  • Coordinate with clinical staff to ensure timely initiation of care.
  • Maintain accurate documentation and records in compliance with regulatory standards.
  • Communicate effectively with clients and families to explain services and answer questions.
  • Collaborate with insurance providers and verify coverage.
  • Ensure smooth transitions from referral to active service.


Requirements

Qualifications:

  • Minimum 1-3 years of experience in a home health or healthcare intake/referral role.
  • Some understanding of Medicare Medicaid and private insurance processes.
  • Excellent organizational and multitasking skills.
  • Proficient in Microsoft Office and electronic health record (EHR) systems.
  • Bilingual in Spanish is preferred but not mandatory.
  • High school diploma required; associate or bachelors degree in healthcare administration or related field preferred.
