About the Role
The ED Care Coordinator through screening and recruitment in the Emergency Department (ED) is responsible for the delivery of timely and comprehensive assessment and care coordination of patients with complex needs to facilitate the allocation of hospital and community resources in an efficient and cost-effective manner. Assessment and care planning are achieved in collaboration with patients their carers and other members of the Emergency Department Clinical team. This role also includes recruiting clients onto the program in a community case management capacity conducting comprehensive assessments coordinating all aspects of health and social care needs monitoring achievement of client goals facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.

Classifications for this position will be Allied Health Grade 2

($90261 - $103724 per annum pro rata for part time).

About ALERT (Healthcare for the Homeless)

This position is part of the Health Care for homeless Program (HfH). The program has a base of services within St Vincents that currently support clients that are experiencing or at risk of homelessness. This service aims to build on integrating and better coordinating existing services within St Vincents hospital and the development of new services to meet the need of the target client group.

Operationally the role has strong links with the Health Independence Program (HIP). HIP offers a comprehensive range of specialist clinics services and rehabilitation programs providing rapid response health management short-medium term care coordination and health education. HIP enable clients to be the decision makers of their care and support clients through self management to achieve their healthcare goals.

ALERT key target groups include patients with complex needs who fall under one or more of the key risk factors; substance use mental health issues homelessness disability and complex aged care issues family violence.
This position is based at St Vincents Hospital Melbourne within the Emergency Department and also providing support and interventions in the community as part of an interdisciplinary team.

4. POSITION PURPOSE
The ED Care Coordinator through screening and recruitment in the Emergency Department (ED) isresponsible for the delivery of timely and comprehensive assessment and care coordination of patientswith complex needs to facilitate the allocation of hospital and community resources in an efficient andcost-effective manner. Assessment and care planning are achieved in collaboration with patients theircarers and other services involved with the clients care within the organisation and community role also includes recruiting clients onto the program in a community case management capacityconducting comprehensive assessments coordinating all aspects of health and social care needsmonitoring achievement of client goals facilitating intervention from other health care professionals andensuring appropriate linkages with hospital and community services.


5. POSITION DUTIES

  • Early identification of ALERT patients in the Emergency Department.
  • Maximize a patients functional independence through assessment and treatment planningimplementation and evaluation.
  • Provide appropriate discipline specific clinical management discharge planning & communityliaison for all relevant patients.
  • To engage with and provide outreach care co-ordination services for identified or referred clientsof the Emergency Department post discharge including ensuring holistic assessment andcollaborative care planning to guide interventions
  • Effective negotiation and advocacy with relevant service providers to ensure optimal andcoordinated client care according to best practice principles
  • Ensuring appropriate use of brokerage funds
  • Active participation in team interface processes service development evaluation and portfolioactivities
  • Perform within the Health Independence Program (HIP) guidelines and specific service guidelines
  • Ensuring all documentation including client records are completed and managed in a timelymanner consistent with SVHM and Complex Care policies and procedures legislative andregulatory requirements
  • Contributing and participating in Complex Care and ALERT professional development (PD)programs and other PD that supports own learning objectives for this role.

6. INCUMBENT OBLIGATIONS

8. SELECTION CRITERIA
8.1 ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS

  • Allied health qualification relevant to ALERT and minimum 2 years experience in relevantacute/community setting.
  • Eligibility for membership of relevant discipline association
  • Current Victorian Drivers Licence

8.2 OTHER ESSENTIAL REQUIREMENTS

  • Experience with prioritizing patients and hospital demands
  • Demonstrated sound knowledge of the application of care coordination practice frameworks andprinciples in the context of the current health care environment.
  • Awareness of Department of Health HIP initiatives and National Health Reform initiatives
  • Demonstrated ability to work autonomously and as part of an interdisciplinary team
  • Demonstrated ability to effectively work with relevant service providers to achieve bestoutcomes for clients
  • Proven ability to effectively manage and negotiate solutions to complex problems
  • Demonstrated commitment to and experience in the development implementation andevaluation of program activities
  • Highly developed interpersonal skills that incorporate the qualities of excellent communicationnegotiation organization planning and conflict management
  • Excellent time management skills including planning organizing and prioritizing of workload
  • Current Victorian Drivers License
  • Participation in roster which includes weekends Public Holidays and afternoon shifts.

Postgraduate qualifications in related field
