Job Summary
Lead your team in making a real difference supporting older people to live well at home.
Were now seeking a Care Partner Leadto join our Community Services team at Marion. This is an opportunity to take the next step in your community services or care management career leading and mentoring a team with empathy purpose and professionalism to ensure all services meet program guidelines budget expectations and client goals.
Responsibilities:
- Providing Leadership Mentoring and Support to Care Partners
- Ensure compliance with the CHSP program guidelines and expectations
- Coordinating and managing individual care plans and service delivery
- Monitoring budgets and outcomes to ensure high-quality person-centred care
- Building supportive relationships with clients families and community networks
- Supporting Home Support Workers and contributing to a positive team culture
- Ensuring compliance with Support at Home program guidelines and Resthavens policies
Essential Criteria:
- Registered Nurse or Allied Health professional with current Australian Health Practitioner Regulation Agency (AHPRA) registration.
- Program Coordination Team Leader case management experience
- Empathy and understanding of issues for older people their families and carers
- A commitment to supporting older people to remain at home in the community
- An ability to work with individuals to identify their needs develop personal goals and develop responsive packages of services
- Knowledge and experience of quality systems for evidence-based care
- Computer literacy
- An SA Drivers License
- The following qualifications are highly regarded: Diploma of Nursing Certificate III in individual Support (Ageing) Certificate III Health Services Assistance Certificate IV in Aged Care Certificate IV in Disability Certificate IV in Community Services Diploma of Community Services (Case Management) Diploma of Ageing Studies and services
