JOB TITLE: IICAPS Counselor

EMPLOYMENT TYPE: Full-Time 40 hours/week

SCHEDULED HOURS: Monday-Friday; Day Shift 1-2 evenings required a week.

ABOUT THE PROGRAM: Intensive In-Home Child and Adolescent Psychiatric Service (IICAPS)

Provides comprehensive community-based clinical case management and case coordination services for children and adolescents aged 3 to 18 who have a history of or at risk of hospitalization. The goals of IICAPS include stabilizing and maintaining children in their homes and communities developing strength-based comprehensive assessments and treatment plans and improving the childs relationships within his or her school family and community.

ABOUT THE POSITION: IICAPS Counselor

Duties & Responsibilities

  • Assists team member in conducting client intake information evaluations assessments referrals and releases of information.
  • Informs clients and their families requirements for program completion.
  • Informs clients of their rights and responsibilities.
  • Provide individual family educational and wellness counseling.
  • Use model tools and complete discharge summaries with clients and families.
  • Understands specific model and uses concepts and model principals when appropriate to aid clients in understanding and managing their symptoms.
  • Provides opportunities for clients and families to practice skills.
  • Maintains family and other collateral contacts to ensure quality client-centered services.
  • Assesses and involves natural supports in treatment planning and discharge processes.
  • Provides case management clinical and crisis intervention supports as indicated. Works in close collaboration with assigned therapist and supervisor in event of crisis.
  • Responsible for arranging aftercare clinical and non-clinical services.
  • Demonstrates competency in topics of child development and provides trauma sensitive and culturally competent care.
  • Uses EHR to document all services and contacts with clients collateral contacts and appointments.
  • Responsible for accurate input of computer data.
  • Participate as a full member of the team and attends all model required meetings and agency trainings.
  • Other duties as related to the position.

QUALIFICATIONS:

Education: Bachelors degree in human services required.

Experience: Experience in working with children who have behavioral health issues and families required. Preferred experience working in the home and community environment at least two (2) years experience in a mental health setting.

Licensure/Certification/Registration: Valid drivers license.

Training: Will attend all required program trainings.

COMPENSATION*:

$20.65/hour and up commensurate upon experience.

*Actual rates are determined at the time of offer and are based on relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note for part-time positions this range would be pro-rated based on hours worked)
