SWAMS is offering an exciting career opportunity for a Registered Nurse/Allied Health professional who is passionate about chronic conditions management. This role would suit someone with experience in the self-management of clients with cardiac renal or diabetes conditions. This position provides culturally appropriate and holistic primary health care to our clients in a primary health care setting. The successful candidate will be required to assist GPs with the development of GP management plans and act as a care coordinator to liaise with hospitals primary care and allied health services to maximise health outcomes for clients. This position can include travel throughout the South West during working hours using a work car. This role is supported by a multidisciplinary primary health team.
Even though a registered nurse is preferred applications from Allied Health professionals with experience in managing chronic conditions or diabetes educators will be considered.
Specific requirements of this position include but are not limited to:
Essential Criteria
(AHPRA) Registered or Enrolled Nurse with at least 3 years post-graduation experience in nursing.
Practical knowledge of principles and models of health behavior change and facilitation of chronic condition self-management skills
Current working knowledge of local and regional hospitals community and primary health systems including services involved in the provision of chronic disease management
Demonstrated organisational and time management skills along with an ability to adapt to changing needs
Proven ability to use clinical software and electronic health records effectively with extensive experience in developing implementing and reviewing care plans for patients with chronic conditions
A demonstrated understanding of the unique issues effecting and impacting upon the health of Aboriginal people
Desirable Criteria
Experience in Community or Aboriginal Health
Hold current immunisation certificate
Venipuncture competency and currency of practice
Experience and interest in Community or Aboriginal Health
Knowledge and expertise in the use of Commuincare or similar clinical database system
