Join Adelaide Hills Community Nursing team in the picturesque Adelaide Hills as a Clinical Nurse specialising in Palliative Care. This ongoing part-time position (30.4 hours per fortnight) offers the opportunity to make a meaningful difference in patients lives during their most vulnerable moments. Working within the Barossa Hills Fleurieu Local Health Network you will provide compassionate person-centred clinical nursing care to patients and their families. HOW WILL YOU CONTRIBUTE

• Deliver proficient personcentred palliative nursing care and case management across home hospital and community settings ensuring safe evidencebased care for clients and their families.

• Assess plan implement and evaluate individualised care plans coordinating with the multidisciplinary team to achieve the best possible clinical and psychosocial outcomes.

• Provide leadership support and guidance to Enrolled Nurses students and less experienced staff fostering a positive learning culture within the palliative care team.

• Contribute to continuous quality improvement by participating in service development clinical audits and practice review and by taking part in the palliative care oncall roster as required.

• Qualifications relevant to palliative care are desirable.
