Join Adelaide Hills Community Nursing team in the picturesque Adelaide Hills as a Clinical Nurse specialising in Palliative Care. This ongoing part-time position (30.4 hours per fortnight) offers the opportunity to make a meaningful difference in patients lives during their most vulnerable moments. Working within the Barossa Hills Fleurieu Local Health Network you will provide compassionate person-centred clinical nursing care to patients and their families. This role combines clinical expertise with genuine human connection ensuring dignity and comfort for those in your care. The Barossa Hills Fleurieu Local Health Network runs from Kangaroo Island to Eudunda wrapping all the way around our metropolitan areas (position based in Mount Barker). HOW WILL YOU CONTRIBUTE

  • Deliver proficient personcentred palliative nursing care and case management across home hospital and community settings ensuring safe evidencebased care for clients and their families.

  • Assess plan implement and evaluate individualised care plans coordinating with the multidisciplinary team to achieve the best possible clinical and psychosocial outcomes.

  • Provide leadership support and guidance to Enrolled Nurses students and less experienced staff fostering a positive learning culture within the palliative care team.

  • Contribute to continuous quality improvement by participating in service development clinical audits and practice review and by taking part in the palliative care oncall roster as required.

  • Qualifications relevant to palliative care are desirable.
