At Pathways Hospice (Newest Start Up) our associates are our most valuable asset. Each associate provides a service that directly or indirectly affects patients families other team members and the public. We strive to excel in providing the highest quality healthcare and customer service possible which is incumbent upon the excellence and commitment of our leaders and caregivers.
We are located in HoustonTX (HarrisCounty). Ultimately we will serve the vast majority of Harris County initially our service will start in the East and Southeast Region of Houston at initial phase of service. (Baytown LaPorte Clear Lake etc.)
We are excited to be able to contribute to this grow in this amazing healthcare direct-community with our newly formed company contributing to all patients with a continuum of care and long-term care services. It is our goal to DELIVER excellent and patient experiences to every patient and resident family member.
Hospice CNA(Certified Nursing Assistant) - PRN
Service Area: Southwest
Responsibilities:
- Provide exceptional care in coordination with the assigned nursing care team.
- Maintain patient records that include any changes in doctors orders diet or personal living activities.
- Help with patient meals feeding and basic activities of daily living.
- Deliver weekly reports to the case manager and review any important changes in patient conditions.
- Educate patients and family members.
- Prepare timely complete and accurate clinical notes.
- Communicate effectively with healthcare providers and other care team members.
Qualifications:
- CNA license in the state of Texas (required).
- One (1) year hospice experience.
- Able to lift up to fifty (50) pounds.
- Able to work occasional weekends holidays evenings.
- Strong communication and people skills.
- Experience withHCHB EMR (preferred but not required)
