Provides episodic clinical and medical care for patients seen in the Community Health settings. Consults with other physicians regarding treatment and management of community patients. Participates in Plan of Care development evaluation and revision. Interacts with various adult patient populations. Provides medical care health screenings and education to community patients.
- Completes physical assessments of patients specific to the patients chief complaint for the episodic visit.
- Establishes a medical diagnosis and plan for the short-term problem or the exacerbation of a chronic condition.
- Orders and interprets results of laboratory and/or diagnostic procedures.
- Develops and documents a medical plan of care for each patient in accordance with results of the patients physical assessment laboratory and diagnostic tests.
- Prescribes appropriate medications for the treatment of common short-term illnesses and chronic stable diseases.
- Refers patients to appropriate licensed physicians and other health care providers for needed consults and additional medical treatment.
- Demonstrates interpersonal skills appropriate to the age/developmental stage of the patient and family.
- Adheres to established policies and procedures.
Qualifications :
- Current Maryland Nurse Practitioner license as an adult geriatric or family nurse practitioner or current Maryland Physician Assistant licensure.
- 2 years experience as a Nurse Practitioner or Physician Assistant in a hospital or ambulatory practice setting.
- Competent in providing comprehensive physical assessments and establishing medical diagnosis for common short-term or chronic stable health problems.
- Ability to work well in a team-oriented environment.
- Possesses excellent communication and negotiation skills.
- Able to provide own transportation.
