Responsibilities:
Provide direct patient care under the supervision of a Registered Nurse or physician
Administer medications and treatments as prescribed
Monitor patient health and report changes in condition to the clinical team
Document all care provided in accordance with facility and regulatory standards
Support patients with activities of daily living and promote comfort and safety
Collaborate with interdisciplinary staff to ensure quality patient outcomes
Qualifications:
Active LPN license in the state of New Hampshire
Current CPR/BLS certification
Strong communication teamwork and clinical skills
Minimum of 1 year of experience in a skilled nursing or long-term care setting required
