_{Providence Health and Services in Medford OR is seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) to join their Social Work Case Management this vital role youll support patients with complex psychosocial needs including mental health assessments behavioral health placements and social determinants of health (SDOH) navigation.}

_{Responsibilities:}

_{Conduct comprehensive risk and psychosocial assessments across hospital units.}
_{Develop and implement discharge plans and provide clinical social work services.}
_{Coordinate mental health and substance use disorder (SUD) placements.}
_{Provide patients with access to community resources related to SDOH.}
_{Collaborate with physicians nurses and case managers to ensure coordinated care.}
_{Float to other units as needed to meet staffing demands.}
_{Work variable weekends based on departmental PTO coverage.}

_{Requirements:}

_{License: Active Oregon LCSW license (must be active at time of submission no exceptions).}
_{Experience: Minimum 2 years of recent clinical social work experience in a hospital or acute care setting.}
_{Caseload: Comfortable managing caseload ratios of 1:40.}
_{Systems: Proficient in Epic EMR charting.}
_{Travelers: First-time travel clinicians welcome.}
_{Other: Experience with risk assessments psych/SUD placements and resource coordination is required.}

_{Qualifications:}

_{Education: Masters Degree in Social Work (MSW) from an accredited institution.}
_{Certifications: Active Oregon LCSW license; BLS not required.}
