Travel Social Worker - LCSW
Location: Camden ME
Shift: 5x8 Days 08 am to 4 pm
Duration: 14 Weeks
Guaranteed Hours: 40
Hourly Pay: $44/hr
Key Responsibilities
- Conduct initial and ongoing psychosocial assessments to identify residents emotional social and environmental needs
- Collaborate with interdisciplinary care teams to develop and implement individualized care plans
- Coordinate complex discharge planning including community referrals home health services and durable medical equipment
- Provide short-term counseling and crisis intervention to residents and families
- Advocate for resident rights and assist with advance directives and power of attorney matters
- Support residents and families with financial resource guidance including Medicaid and insurance navigation
- Maintain accurate timely documentation in compliance with state and federal regulations
Requirements: - Licensure: Active Social Work license in the State of Maine (LSW LMSW or LCSW required)
- Education: Bachelors degree in Social Work (BSW) required; Masters degree (MSW) preferred
- Experience: 1-2 years of recent healthcare social work experience preferred (SNF Long-Term Care or Acute Care)
