Job Summary

Travel Social Worker - LCSW

Location: Camden ME

Shift: 5x8 Days 08 am to 4 pm

Duration: 14 Weeks

Guaranteed Hours: 40

Hourly Pay: $44/hr

Key Responsibilities

Conduct initial and ongoing psychosocial assessments to identify residents emotional social and environmental needs
Collaborate with interdisciplinary care teams to develop and implement individualized care plans
Coordinate complex discharge planning including community referrals home health services and durable medical equipment
Provide short-term counseling and crisis intervention to residents and families
Advocate for resident rights and assist with advance directives and power of attorney matters
Support residents and families with financial resource guidance including Medicaid and insurance navigation
Maintain accurate timely documentation in compliance with state and federal regulations

Requirements:

Licensure: Active Social Work license in the State of Maine (LSW LMSW or LCSW required)
Education: Bachelors degree in Social Work (BSW) required; Masters degree (MSW) preferred
Experience: 1-2 years of recent healthcare social work experience preferred (SNF Long-Term Care or Acute Care)
