Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and detail-oriented Long-Term Care Functional Screener to join our IRIS Consulting Team in Milwaukee WI! ð
In this role youll help individuals enrolled in the IRIS Program (Include Respect I Self-Direct) determine their eligibility for publicly funded long-term care services. Youll be part of a mission-driven team that empowers people to live independently and meaningfully.
ð What Youll Do
- ð Build trusting relationships with IRIS participants
- ð Administer Long-Term Care Functional Screens (LTC FS) annually or as needed
- ð Coordinate and document LTC FS renewals
- ð Provide guidance to help participants maintain funding eligibility
- ððð Collaborate with ICA team members and community partners
- ð Participate in staff meetings and multidisciplinary conferences
- ðâï Prepare reports and may testify in hearings or reviews
- ðª Ensure compliance with federal and state documentation standards
- ð Strengthen community connections to support participants

ð Qualifications
- ð Bachelors degree in social work psychology human services nursing special education or related field
- ðð½ Minimum 1 year of experience working with seniors or individuals with disabilities
- â Valid LTC FS Certification (or willingness to obtain)
- ðª Strong commitment to participant autonomy and meaningful living
- ð Comfortable with smartphones tablets and computer systems
- ð Knowledge of human behavior social dynamics and community resources
- ð Strong organizational and planning skills
- ð Ability to work independently and collaboratively
ð Requirements
- Valid drivers license & reliable vehicle
- Satisfactory driving record (MVR check required)
- Must pass Caregiver Background Check
- Willingness to travel frequently (day trips occasional overnight)
