Job Summary
Job Description
Service Description:CONTRACTOR shall provide qualified NYS licensed physical therapist(s) to perform physical therapy consultant services onsite for Bronx Psychiatric Center (BPC) clients/ patients. The services will be provided for an estimated 20 hours per week. The NYS licensed physical therapist shall:
A. Act as physical therapy consultant.
B. Assess and evaluate clients condition in order to develop a physical therapy treatment plan based upon a physicians request.
C. Review clients treatment goals treatment and medical history.
D. Perform and interpret standard muscle nerve joint and functional ability tests.
E. Administer appropriate physical therapy treatment when indicated.
F. Prepare documentation of assessment evaluation delivery of treatment services and patient progress for clients medical record.
G. Consult with clients treatment team as needed.
H. Be under the supervision of Bronx Psychiatric Center Director of Medicine or the Directors designee.
I. Conduct a brace/shoe clinic as needed nerve joint and functional ability tests.
E. Administer appropriate physical therapy treatment when indicated.
F. Prepare documentation of assessment evaluation delivery of treatment services and patient progress for clients medical record.
G. Consult with clients treatment team as needed.
H. Be under the supervision of Bronx Psychiatric Center Director of Medicine or the Directors designee.
I. Conduct a brace/shoe clinic as needed.
J. Evaluate patients for wheelchairs and adaptive equipment.
K. Submit to the Medical Director and Business Office: monthly statistical information including number of patients treated number of physical therapy modalities administered number of evaluations and number of
admissions/discharges in the physical therapy services.
L. Maintain a performance improvement program related to structure process and outcomes.
