GENERAL DESCRIPTION:

Those mentors were life-changers and now with this Transition Navigator position you can make a similarly positive impact!

Our team of dedicated counselors and staff working at local regional and statewide levels is passionate about enhancing the lives of youth with disabilities. As a Transition Navigator you will assist counselors in one of four regions across the state delivering high-quality vocational rehabilitation services Pre-Employment Transition Services (PreETS) for school-attached youth aged 14 to 21 who have a disability. Your responsibilities will include managing the intake and orientation process for Potentially Eligible and general Division of Vocational Rehabilitation (DVR) services performing initial assessments and determining whether a student needs the services of a VR counselor or basic case management services. Additionally you will collaborate with various school districts to recruit students
conduct group activities and monitor student progress. Each regionRock Springs Lander Cheyenne and Gillettehas unique needs but the core mission of this role is to help students with disabilities explore their future.

This position demands extensive travel within your assigned region and close collaboration with DVR counselors school districts students and parents. If you are excited about helping students discover their interests and plan for their future we encourage you to apply.

  • Recruiting Eligible Students: Identify and enroll Potentially Eligible students for Pre-ETS services through local school meetings Community College Career Readiness Programs and other points of contact.
  • Outreach Activities: Promote DVR Pre-ETS services through various outreach efforts.
  • Collaboration: Work closely with students parents stakeholders school districts community colleges and the local VR counselors.
  • Career Exploration: Provide creative and innovative career exploration opportunities for students through workshops orientations group and individual activities.
  • Employer Networking: Build relationships with local employers to create student work experience opportunities and provide training for employers regarding disability topics.
  • Case Management: Offer basic case management to ensure students have access to and referrals to services. This includes completing and gathering forms collecting documentation and providing information and keeping accurate records of participants served and what service they received.
  • Mentorship: Serve as a mentor providing support encouragement and inspiration to students as they explore their futures.

Qualifications


PREFERENCES:

Preference for a Bachelors degree in a human service-related field

Preference may be considered for an Associates degree in the human service field and extensive experience

Preference for one year of experience working with youth in transition or providing Pre-Employment Transition Services tostudents with disabilities

Preference may be considered for equivalent combinations of related education and experience.



KNOWLEDGE:

  • Knowledge of IEPs 504 Plans and regulations
  • Ability to work in cooperation with Local Education Agencies DVR and Workforce office
  • Ability to be proactive organized and manage caseloads
  • Knowledge of informed choice confidentiality and conflict of interest -Knowledge of disability rights culture and history
  • Skill in working with individuals with disabilities as well as skills in sensitive respectful and effective communication with people of diverse backgrounds cultures and languages
  • Skills in public speaking
  • Ability to work within your scope of practice knowledge skill and abilities and an ability to delegate refer or communicate when a duty or role is outside of your scope of practice or skill level


MINIMUM QUALIFICATIONS:

Education:
Bachelors Degree
Experience:
0-1 year of progressive work experience (typically in Benefits and Eligibility)
OR
Education & ExperienceSubstitution:
3-4 years of progressive work experience (typically in Benefits and Eligibility)
Certificates Licenses Registrations:
None
