Pinnacle Home Care Floridas largest Medicare-certified home health provider has been delivering high-quality patient-centered care for over two decades and were looking for a Referral Outcomes Coordinator to join our award-winning team.

Key Responsibilities:

  • Accurately process non-admits in accordance with company policies and compliance requirements.
  • Document all non-admit decisions and rationale in the appropriate systems (e.g. Allscripts Kantime internal trackers).
  • Collaborate with clinical and operational teams to ensure all documentation is complete before case closure.
  • Track and monitor non-admits to identify trends and escalate concerns to leadership.
  • Maintain organized up-to-date records for auditing and reporting purposes.
  • Provide timely updates to Regional Directors Referral Development Specialists (RDS) and other stakeholders.
  • Support reporting processes by contributing to daily weekly and monthly non-admit metrics.
  • Participate in team meetings and contribute to process improvement initiatives.

Qualifications:

  • High school diploma or equivalent required; associates or bachelors degree preferred.
  • Prior experience in healthcare administration patient intake or referral management strongly preferred.
  • Familiarity with EMR/EHR systems a plus.

  • Strong attention to detail and accuracy in documentation.
  • Excellent written and verbal communication skills.
  • Ability to manage competing priorities and meet deadlines.
  • Strong organizational and problem-solving skills.
  • Ability to work independently and as part of a team in a fast-paced environment.
