Working in the Individuals home the Skills Trainers role is to provide support that will allow the Individual to gain access to their community. Individuals in the program range in age from 6-22 and are students currently enrolled in a school-based as part of a family team and under the guidance of a family initiated and supervised goals the Skills Trainer will provide support and participation in leisure and recreational activities and may also carry over skills learned in school. The Skills Trainer will work with the Individual to identify appropriate activities and provide support for with individuals and families to achieve goals identified by the Individual family and Family Support and assistance with the participantComplete all necessary documentation to assure compliance with Communitas and DDS policies (documentation is very limited)Ensure the rights and dignity of all people supported is upheld at all timesDevelop and maintain supportive working relationships with program membersMaintaining strict confidentiality for all information and facility operationsProvide personal care and assistance to individuals consistent with individual needs as they strive to reach their goalsCheck out our website:
