PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Care Coordination Services team as the Program Manager C3RES.

ABOUT PATH
Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.

ABOUT THE JOB
The program manager is responsible for the programs day-to-day operations and supervision of service delivery staff. The incumbent will collaborate with both internal and external departments to enhance accessibility and service delivery for program participants. The incumbent is responsible for developing and ensuring adherence to programmatic policies and procedures. The incumbent will also be responsible for identifying and providing trainings. Position Responsibilities include:

  • Provide clinical oversight of staff and ensure that all program participants receive the services as identified in each participant service plan.
  • Create and work within a team environment to ensure staff is knowledgeable and familiar with contract expectations case management and supervisorial expectations and ensure staff is delivering high quality care.
  • Provide individual and team supervision support and performance evaluations for assigned staff
  • Ensure that services emphasize trauma informed care client safety and harm reduction interventions.
  • Be available to respond to emergency calls and situations as required by the contract and organization
  • Assist in the screening assessment and enrollment of clients in a range of services

PREFERRED QUALIFICATIONS

  • Current and active licensure with the California Board of Behavioral Sciences (e.g. LCSW LMFT LPCC).
  • Three (3) years of experience in healthcare case management homeless services or working with vulnerable populations.
  • One (1) year of supervisory experience in homeless services or related social services field strongly preferred

MINIMUM QUALIFICATIONS

  • Masters degree in social work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences (e.g. MSW MFT)
  • Two (2) years of experience in healthcare case management homeless services or working with vulnerable populations

MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:

  • Have reliable transportation
  • A valid drivers license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATHs insurance coverage
