POSITIONSUMMARY

The Aftercare Specialist plays a vital role insupporting youth as they transition from residential treatment back into theircommunities. Working closely with residents during their stay thispositionis responsible fordeveloping individualized aftercare plansthat promote long-term recovery stability and success. The AftercareSpecialist collaborates with families tribal representatives probationofficers and community resources to ensure each youth hasa strongsupportnetwork and access to ongoing services. This role requiresexcellent communication documentation and follow-throughtomaintaincontinuity of care and track youth progresspost-discharge.

ESSENTIALDUTIES AND RESPONSIBILITIES

Build trustingrelationships with youth during residential treatment to develop personalizedaftercare plans.

Work in closecollaboration withtheresidents primary counselor to design acomprehensive culturally responsive aftercare plan that aligns with treatmentgoals and supports long-term success.

Coordinate withfamilies tribal representatives probation officers and community providerstoidentifyculturallyappropriateand trauma-informedresources.

Maintain regularcommunication with youth andfamiliespost-dischargetomonitorprogress.

Updateandmaintaina comprehensive resource guide for community servicesincluding housing education employment mental health and substance usesupport.

Document all casemanagement activities communications and progress notesin accordancewithagency and regulatory standards.

Track youth outcomesand engagement with aftercare services to inform program improvements andreporting.

Participatein multidisciplinary team meetings to ensure continuity of care and alignmentwith treatment goals.

Advocate for youthneeds and help navigate barriers to accessing services in their homecommunities.

Respond to crises orsetbacks with compassion and solution-focused supportinvolvingappropriate partiesas needed.

Upholdprofessionalism respect and confidentiality in all interactions with youthfamilies and external partners.

Provide support andbackup coverage forgroupsand/or intake functions.

Demonstrate flexibilityand responsiveness in fast-paced situations including unexpected discharges orurgent transitions ensuring aftercare planningremainsthoroughandtimely.

Assistwithtransitionallogistics which may includehelpingresidentsmanage items such as cell phones or other toolsthat support their reintegration and communication needs.

Someparticipationin community networking events to build relationships withexternal service providers strengthen referral pathways and promote awarenessof available resources.

May be present duringyouth discharge including outside of standard business hours to supportsmooth transitions and ensure aftercare plans are activated effectively

MINIMUMQUALIFICATIONS

Candidatesmust meet the minimum educational and experience requirements for the positionanddemonstrateproficiencywith Microsoft Office Suite(Outlook Word Excel PowerPoint) and other relevant applications.

Bachelors degree inSocial Work Counseling or otherrelated fieldrequired OR willing to consider a combination of experience and/or educationthatdemonstratesability to perform successfully in the position.

Minimum of 2 years incase management youthservicesor behavioral health.

Knowledge of adolescentdevelopment trauma-informed care and substance use recovery

Familiarity with tribalcommunities probation systems and culturally responsive practices.

Strong communicationorganization and documentation skills.

Ability to workindependently and collaboratively in a fast-paced residential setting.

ProficiencyinMicrosoft Office and electronic health record systems.

Valid drivers license

Experienceinfacilitatingtherapeutic groups preferred


PHYSICAL DEMAND
While performing the duties of this position the employee maybe required to stand walk or sit for extended periods; bend stoop orcrouch; lift and/or move up to 10 pounds; use hands to handle or feel objectsand reach with arms; and communicate clearly and effectively in person and byphone. Specific vision abilities include close vision and the ability to adjustfocus. Occasional extended hours or weekend work may berequired.

WORK ENVIRONMENT
Work is typically performed in an indoor collaborative noise level is moderate and interactionsfrequentlyinvolvecommunication and teamwork beyond basic work instructions.
