Job Summary

General Summary:

This is a direct service position requiring general knowledge of serious mental illness and specific skill in rehabilitation with persons with disabilities. The position requires knowledge of medication management. This is an hourly position. The Medication Coordinators are supervised by the Program Supervisors with clinical supervision provided by a manager.

Principal Responsibilities and Duties:

1. Provide medication monitoring according to policy. Assist in training of staff in medication monitoring process.

2. Provide transportation for members.

3. Assess for potential crisis and implement crisis intervention actions as appropriate and according to proper policy and procedures.

4. Promote and ensure the health safety security and organization of the work environment.

5. Provide information to the teams problem solve and implement member plans as directed by the team.

6. Maintain current accurate and complete documentation related to all service functions.

7. Initiate communication with supervisor regarding concerns ideas input suggestions or problems.

8. Manage and record changes to medication sheets and plans.

9. Provide communication support and linkage needed to help members keep medical appointments both psychiatric and somatic.

10. Provide education and linkage to new and existing members regarding medication and health services.

11. Assist members in communicating with healthcare providers both somatic and psychiatric. Develop medication plans on all members receiving services.

12. Meeting with members in their homes and other facilities to work side by side with them in cleaning completing chores cookingetc.

13. Provide direct service to members needing assistance with medication monitoring including assessment and education.

14. Conduct audit reviews of lock-boxes and audits of the med monitoring process of staff in supported and/or residential houses as scheduled.

15. Provide and/or assist with trainings (on policy medication plans and specific medications or procedures) for peers as assigned.

16. Educate members regarding the effects of their medication.

17. Use computerized system to update and maintain medication monitoring plans.

18. Update computerized medication list under supervision of RN.

19. Manage pharmacy checking in meds.

20. Schedule members to fill pillboxes.

21. Assure accurate timely ongoing availability of medication for members from written prescription to member pill box.

22. Perform duties in Day Program Nursing Office such as filling weekly medication supplies monitor daily medications.

23. Ensure members have medical updates for all appointments.

24. Perform other duties as assigned.

Knowledge Skills and Abilities Required:

1. Possession of a high school diploma or GED. Bachelors degree preferred.

2. Positively referenced relevant work volunteer or student experience.

3. Valid drivers license and safe driving record.

4. Possession or use of a four passenger car to be used to transport clients and a copy of a valid certificate of automobile insurance.

5. Must be covered under an active auto insurance policy and a valid copy of auto insurance is required prior to hire.

6. Ability to establish a positive peer relationship and to coordinate effective rehabilitation services.

7. Ability to communicate Sheppard Pratts mission when interacting with neighbors families and external agencies and providers.

8. Knowledge of medications (all medications but specifically psychotropic) side effects and therapeutic effects and the ability to effectively communicate this knowledge.

9. Ability to work as part of an integrated interdisciplinary team.

10. Knowledge of community resources and skill in linkage and advocacy.

11. Ability to learn psychiatric rehabilitation concepts.

12. Ability to establish and maintain empowering trusting relationships with members and their families.

13. Ability to maintain appropriate relationship boundaries and to take corrective action when needed.

14. Ability to communicate orally and in writing.

15. Ability to obtain assistance with decision making appropriately
