PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Site Based Housing team as the P/T Activities Coordinator at Jacaranda office

ABOUT PATH
Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.

ABOUT THE JOB
The Activities Coordinator will offer activities in a permanent supportive housing setting for individuals integrating evidenced-based practices effective with tenants who have experienced homelessness and have a history of chronic health mental health and/or co-occurring conditions. They will develop and lead residential activities and workshops develop relationships with community partners and volunteers coordinate activities schedule community outings for residents greet clients as they enter the facility assist with signing clients up for orientations and workshops and educate clients and other programs on current rules and referral processes.

Shift: Monday Wednesday Friday; 8:00am - 4:30pm

Position Responsibilities include:

  • Develop and lead workshops and activities designed to assist clients in maintaining stable housing and integrating into the VV community in the areas of but not limited to: independent living skills housing stabilization money management community integration employment benefits establishment linkages to community providers for problematic substance use primary and mental health care and all other services needed to assist clients in reaching their treatment plan goals.
  • Perform initial needs and interest surveys to ensure activities are tailored to the specific needs of the residential community.
  • Create a monthly calendar of activities including programming at least 2x/week including at least 1 community integration outing 1 guest speaker and 1 skill building class (health living recovery social enterprise etc).
  • Schedule residential outings coordinating with community partners and volunteers

PREFERRED QUALIFICATIONS

  • Bachelors Degree in a related field.
  • Two (2) years of experience in healthcare case management homeless services or working with vulnerable populations or a related field

MINIMUM QUALIFICATIONS

  • All levels of education and experience welcome.

MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:

  • Have reliable transportation
  • A valid drivers license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATHs insurance coverage
