Job Summary

2 years inpatient discharge planning experience is experience strongly weekends and float to different : approximately 1000 bed Level I Trauma of approximately 16 to 24 cases per - Epic Case Management : Business casual and white lab coatHospitalist or Intensivist in house 24/7RT on site- 24/7Pharmacy on site- 24/7Shift and Weekend expectations (i.e. 6a-6p; Every other weekend- Floating required)Holiday and on-call expectations (i.e. on call 3X per week; holiday - 1 major 1 minor)occasional weekend and holiday
