Job Summary:
This is an 18-week contract position for a Licensed Clinical Social Worker (LCSW) experienced in Hospital Care Management and discharge planning. The LCSW will be responsible for providing social work services care management and discharge planning within a hospital setting. The contract is expected to continue into January with the earliest end date being March 30.
Location:Newberg Oregon United States
Responsibilities:
- Provide care management services.
- Perform discharge planning.
- Collaborate with the healthcare team.
- Perform data entry into electronic medical database systems.
- Document patient care and related duties appropriately.
- Perform selected patient registration activities.
- Coordinates equipment/supply needs. Notifies appropriate department of equipment malfunction.
- Perform any other related duties (e.g. clerical technical duties) as assigned.
Required Skills & Certifications:
- Active LCSW license in the state of work.
- Experience in Hospital Care Management.
- Experience in discharge planning.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Travelers may request standard and/or holiday time off (only one of the three major holidays); however approval is at the clients discretion.
- No floating between units.
- No on-call required.
- Weekends are required (2 weekend shifts every pay period).
- No block schedules required.
- Ratios vary.
- Contract must continue into January.
