Job Title: Social Worker - MED/SURG


Social Work Services at the Mount Sinai Hospital


The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.

  • This position involves the provision of social work services to cardiology patients in the Heart Hospital.


Responsibilities
  • Responsibilities include coverage of patients on social worker will be responsible for providing psychosocial assessments and psychosocial services for patients especially helping patients and families cope with the impact of hospitalization and of acute and chronic illness.
  • There is also a strong emphasis on complex discharge planning which requires fast-paced coordination of home care services and facilitation of placements.
  • Coverage responsibilities will be on other cardiac units.
  • The social worker is an integral part of the Heart Hospital and has responsibilities that include teamwork committee meetings and participation beyond the purview of the floor assignments.
  • Knowledge of crisis family and systems theories is utilized in an interdisciplinary setting.



Qualifications
  • NYS LMSW or LCSW.
  • Ability to develop and implement safe appropriate and timely discharge plans in a demanding fast-paced environment.
  • Skills in assessment and coordination of resources within hospital and community networks for effective management of patient care.
  • Excellent organizational collaborative and psychosocial assessment skills.
  • Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care.
  • Inpatient hospital experience preferred.
  • Minimum one year in current position unless currently a per diem or a float in Cardiac.
