Reporting to the Director Primary Health Care the Equity Diversity and Inclusion Ambassador and Special Projects Lead is responsible for enhancing service coordination and connecting clients to various social services and non-clinical programs needed to manage their health care goals. The Ambassador will work within an inter and multi-disciplinary team of professionals to ensure access to services and programs is progressive to maintain and improve health outcomes. The role will also participate in the assessment care plan and referral process within the clinical team. The Ambassador works to enhance and support clients sense of belonging culture identity and purpose through culturally responsive approaches.
Additionally through capacity building engagement partnership and consultation the incumbent will lead activities and initiatives to integrate and embed anti-racism anti-oppression equity and social accountability principles and practices across all aspects of the agency and in the community in alignment with the organizations vision purpose and values. The incumbent will work closely with senior leadership staff clinicians and patient experience advisors including members of underserved and underrepresented groups to identify diagnose measure and address systemic barriers to equity and inclusion. The incumbent will work in partnership with the CEO to develop and implement a coordinated Equity Inclusion Diversity and Anti-Racism (EIDA-R) strategy and will Lead Projects related to Locally Driven Population Health Models
RESPONSIBILITIES & DUTIES:
- Support day-to-day client requirements including arranging care supports managing complaints and other health care needs.
- Link clients and family members to mental health and addiction supports.
- Build upstream and proactive health care programming for clients that have unmet health care needs. This will include planning and leading outreach activities with community partners and in alignment with Ontario Health funding directions.
- Address health disparities by developing capacity to educate mobilize and engage diverse communities in disease risk reduction (cancer screening chronic disease management
- Build and maintain relationships with partners members of research teams members of equity-deserving groups and other key stakeholders to ensure their perspectives are recognized and valued through all agency activities.
- Develop working relationships and rapport with community service providers in providing culturally adaptive language and services which consider the experiences of the Black Indigenous and Other People of Colour (BIPOC).
- Lead the development and implementation of a revised EIDA-R strategy in alignment with the agencys strategic plan.
- Lead the implementation of the EIDA-R work plan objectives and priorities.
- Lead the agency EIDA-R committee and Client Family Advisory Council (CFAC) .
- Support the education and training plan to enhance EIDA-R knowledge and skills among healthcare staff.
- Evaluate the effectiveness of programming and help prepare key performance indicators and reports for leadership.
- Contribute to the development and renewal of governance documents policies engagement tools and communications.
- Work with HR and Patient Experience on the process to consult on internal investigations relating to Human Rights Code violations.
- Work closely with HR to design and deliver training that builds capacity and understanding of diversity inclusion equity cultural competency anti-racism and anti-oppression.
- Serve as the EDI representative for the agency at external committees and within the Windsor-Essex Health Partners.
- Performs other related duties as required by the team.
EDUCATION & SKILLS:
- Post-secondary education in a related field of social services social work mental health community development or related health discipline OR equivalent combination of education and relevant experience
- Minimum 3 years experience in a community-based setting with emphasis on primary care community development and mental health support and care coordination.
- Experience in client management program development implementation monitoring and evaluation.
- Experience with a wide range of diverse populations e.g. immigrant and newcomers communities racialized groups people who use drugs people living with homelessness mental health and addiction and low-income challenges.
- Experience working in a collaborative and multidisciplinary team environment including primary care and family medicine.
- Experience with service navigation and advocacy for people whove experienced trauma racial trauma and discrimination due to systemic barriers.
- Project management skills and experience leading multiple cross-functional projects.
- Demonstrated knowledge of anti-oppression equity diversity inclusion and anti-racism frameworks and application in organizational settings.
- Demonstrated ability to work collaboratively with staff volunteers and community partners in a multi-disciplinary environment
- Ability to communicate effectively with staff across the organization (including senior management) clinicians external health service providers community-based organizations and customers / vendors.
- Ability to develop relationships with key stakeholders to establish trust credibility and respect.
- Strong analytical and problem-solving skills ability to evaluate alternatives find solutions and make decisions.
- Lived experience as a member of an Indigenous and / or equity-deserving community
- Advanced knowledge of Microsoft Outlook Word PowerPoint Excel SharePoint Teams Project and Visio.
Hours of Work: Thirty-Five (35) hour work week. The agencys regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.
Salary range: $74687.00 $91590.00 annually.
This salary range represents Steps 1 through Job Rate for Band 5 and is inclusive to the maximum earning potential of this role. Compensation within the range will be based on the experience of the incumbent.
Compensation includes a competitive and comprehensive benefit Healthcare of Ontario Pension Plan (HOOPP) vacation and sick leave package.
