Job Summary

Provides supervision of Medicaid Waiver Case Management services by leading and implementing comprehensive proactive consumer-directed systems of support and advocacy for individuals with disabilities and their families ensuring high quality person centered responsive consumer care.

  1. Provide supervision to Medicaid Waiver case management services to Case Managers by ensuring high quality person centered responsive and timely service care with consumers.
  2. Oversees process of referrals data collection and reports coordination of services communications and adherence to county and state guidelines and regulations ensuring services are person-centered.
  3. Hires trains manages and supervises support staff to include Case Managers and Case Management Aids. Performs reviews and coordinates staff schedules to ensure adequate coverage.
  4. Supports Case Managers in professional consumer and/or family issues and needs. Oversees Case Managers processes in consumer services to include planning and accessing housing financial supports maintaining waiver eligibility adherence to governmental guidelines and identification and use of community support services.
  5. Facilitates collaborative problem solving between consumers their families care providers and governmental entities. Maintains professional relationship with all of the above.
  6. Contribute to the financial sustainability of the waiver case management services.
  7. Develops and maintains procedural policies and refines program model accordingly.
  8. Participates in required organizational and governmental agency meetings along with plan lead and facility departmental staff meetings
