Were seeking a Care Navigator to join our mission-driven team supporting seniors in their homes senior living and memory care communities. This role is ideal for someone who enjoys variety values relationships and finds meaning in helping others.
What Youll Do:
- Visit patients in their residential homes to support care plans
- Create individualized care plans to meet the needs of each patient
- Assist with dementia program activities and telehealth visits
- Provide care coordination and resource support
- Build relationships with patients families and community partners
- Support documentation scheduling and community outreach events
What Were Looking For:
- Compassionate adaptable and reliable team player
- Comfortable working independently and in diverse environments
- Medical Assistant diploma preferred
- Experience or education in patient care or case management
- Valid drivers license and ability to travel locally
