Your Role in Our Mission
Easterseals PORT Healths Tailored Care Management (TCM) team is committed to delivering whole-person integrated care that truly makes a difference. As a Tailored Care Manager youll play a vital role in supporting individuals by evaluating their needs developing personalized care plans and coordinating services that address physical and mental health social determinants of health and other life challenges. This is a meaningful hands-on role with the opportunity to travel throughout Beaufort Martin Washington and surrounding counties helping close gaps in care and improving lives in the community every day.

This full-time role offers a primarily MondayFriday schedule from 8 AM to 5 PM with rotating on-call responsibilities. What Were Looking For
Were seeking a compassionate driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after-hours support.
We also require:
- Bachelors degree in a field related to health psychology sociology social work nursing or another relevant human services area. Masters level degree preferred.
- Eligible for Qualified Mental Health Professional (QMHP) credentialing
- Minimum 2 years of experience with mental health and/or substance use populations
- Proficiency in Microsoft Office and Electronic Health Records (EHR)
- Valid drivers license current auto insurance and a clean driving record
- Strong communication skills and the ability to work independently
