Trumen Physicians Associates is seeking a dedicated and experienced Physician to join our growing post-acute care team in Round Rock TX. About the Role:

As a Post-Acute Care Physician youll manage a panel of patients in skilled nursing and rehabilitation facilities. Youll play a key role in reducing hospital readmissions managing chronic and acute conditions and guiding patients through transitional care with compassion and continuity.

Core Responsibilities:

  • Provide in-person medical care to patients in assigned post-acute facilities

  • Perform assessments manage treatment plans and update documentation in the EMR

  • Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care

  • Participate in Trumens value-based care and transitional care programs

  • Develop strong working relationships with facility staff and leadership

  • Serve as a liaison between Trumen and facility teams

  • Be available in assigned facilities anytime during regular business hours on scheduled days

Qualifications:

  • MD or DO with a current unrestricted Texas medical license

  • Board Certified or Board Eligible in Internal Medicine Family Medicine or Geriatric Medicine

  • 1 year of experience in post-acute geriatric or SNF care preferred

  • Comfortable practicing independently in skilled nursing facilities

  • Strong clinical judgment communication skills and a patient-centered mindset

  • Reliable transportation to the facilities
