The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high-quality low-cost innovative community-based ambulatory care. This work stems from Mass General Brighams unified system strategy to bring healthcare closer to patients while lowering total healthcare costs. The Medical Group provides a wide range of offerings including primary care specialty care behavioral and mental health and urgent care both digitally as well as at physical locations in Massachusetts New Hampshire and Maine. The group also offers outpatient surgery and endoscopy imaging cardiac testing and infusion. The Behavioral Health Navigator serves as an integral member of the Social Work Team. This role works closely with clinical social workers physicians advanced practice providers nurses and other clinical and administrative staff to enhance the delivery of patient care services along the continuum of care.

The Behavioral Health Navigator helps support and guide patients through complex social service systems with a goal of promoting social and emotional well-being. With direction from clinical social workers the Navigator will assist patients in identifying and acquiring quality culturally competent behavioral health resources.


Job Summary

Primary responsibilities to include:
Engages patients in the management of their behavioral health conditions through goal-setting coordination with community services and providers and referral services.
Partners closely with clinical social workers and other members of the clinical team to support care plan
Identifies and removes barriers that interfere with patient access to behavioral health resources.
Handles follow-up contact with patient/family care team and community agencies.
Promotes an environment that is sensitive to the culturally diverse needs of patients families and employees.

Research resources and programs available to patients including but not limited to:
Individual group and family therapists
Social opportunities and peer support
Behavioral Health Self-Management Tools

As a member of the Care Coordination Team:
Advocates for the patient to ensure quality outcomes.
Participates in the planning of care for assigned patient population.
Identifies and uses appropriate community resources to optimize patient outcomes.
Finds opportunities for service line process improvement.
Serves as a resource to the interdisciplinary team for community based social service agencies.
Maintains effective working relationships to work collaboratively.
Communicates and collaborates with team to ensure that care coordination needs of assigned patients are met without redundancy of efforts and there is adequate documentation in the patients medical records.


Qualifications

Education
Bachelors Degree Health Administration required and Bachelors Degree Social Work required and Bachelors Degree Related Field of Study required

Licenses and Credentials

Experience
Experience in care coordination case management or patient advocacy 2-3 years required and Experience working with a broad patient population and understanding of community health resources 2-3 years preferred

Knowledge Skills and Abilities
- Strong interpersonal and communication skills to effectively engage with patients families and healthcare providers.
- Knowledge of healthcare systems insurance processes and community health resources.
- Ability to work independently and manage multiple patient cases simultaneously.
- Problem-solving skills to identify and address barriers to care.
- Empathy and cultural sensitivity when working with patients from various backgrounds.
