Shared Living Coordinator
Shared Living Coordinator (ASL Fluency Required)

What Youll Do
- Match individuals with Shared Living providers and support smooth transitions.
- Monitor placements monthly and conduct bi-weekly check-ins.
- Ensure providers meet training standards (CPR/First Aid Human Rights Safety).
- Support individuals in community integration health management and financial oversight.
- Maintain compliance with DDS documentation and ISP requirements.
- Provide on-call support and collaborate with families guardians and funding agencies.
- Directly support up to 12individuals ensuring their health safety and satisfaction.
Qualifications
What Youll Bring
- A related college degree or in lieu of degreea high school diploma with 35 years of relevant experience.
- Familiarity with Shared Living (Adult Foster Care) or residential service models (preferred).
- Strong problem-solving communication and organizational skills.
- Ability to use technology platforms like HCSIS andMAP.
- Knowledge of DDS ISP Process and CARF Accreditation a plus.
- Flexibility empathy and a commitment to person-centered care.
Requirements
- Must pass First Aid/CPR training.
- Successful completion ofbackground checks and requiredtraining.
- Valid Drivers License and access to reliable transportation.
- Obtain Human Rights Officer and Fire Safety Officer certifications.
- Have a vehicle for traveling to home visits and occasional physical activity.
- Willingness to maintain a flexible schedule and attend required trainings.
- ASL Fluent Required
