Job Summary
Position Title: Case Manager
Location: Cape Coral FL 33990 (No local candidates within 50 miles.)
Job Type: Contract Onsite
Shift: Day Shift 8 hours/day 5 days/week 40 hours/week
Pay: $1850$1950 per week
Job Summary:
Navitas Healthcare LLC is seeking Case Managerfor an exciting job in Cape Coral FL.
Responsibilities:
Coordinate and manage patient care plans across the continuum of care.
Collaborate with physicians nurses and interdisciplinary teams for discharge planning.
Evaluate medical necessity and maintain compliance with payer requirements.
Educate and support patients and families regarding post-discharge care.
Float to sister campuses as required.
Maintain thorough documentation and adhere to compliance deadlines.
Qualifications:
Required:
Minimum 2 years of Case Management experience.
Acute care experience required.
Active RN license (Nursys verification required).
Current BLS certification (AHA).
Accredited nursing degree or 5 years of relevant experience.
Willingness to float between facilities.
Preferred:
Previous travel nursing or hospital experience.
