The Health Center Association of Nebraska (HCAN) is actively recruiting a Family Nurse Practitioner to join our Federally Qualified Health Center (FQHC) in Fremont Nebraska where you will provide high-quality patient-centered medical care for all. Utilizing an integrated care approach our providers work collaboratively to address all aspects of patients health and believe that exceptional compassionate care should be accessible and affordable for all. Medical dental behavioral health and outreach services are coordinated to ensure well-rounded comprehensive care.

Your Role:

  • Provide primary care services to patients within professional scope of practice
  • Educate patients on healthy lifestyles and routine health maintenance practices
  • Order and administer diagnostic tests as needed and appropriate
  • Effectively communicate results of diagnostic tests to patients and designated individuals in a timely manner
  • Collaborate with an interdisciplinary team to improve health outcomes
  • Ensure complete and accurate documentation and review of patient records in a timely manner


What We Are Looking For:

  • Masters degree from an accredited school of Advanced Nursing Practice required
  • Valid APRN license in the State of Nebraska required
  • Current DEA license required
  • Knowledge of public health principles and practices
  • Demonstrated professional commitment to providing services to medically under-served individuals in a public health setting
  • Ability to foster strong patient-provider relationships
  • Current certificate in BLS
  • Bilingual English Spanish preferred
