About the Opportunity
As a valued member of our team you will provide high-quality holistic nursing care to patients and their families throughout their healthcare journey. Youll assess health needs develop and implement tailored care plans and work collaboratively with the multidisciplinary team to achieve the best outcomes.
Youll be supported by our Clinical Nurse Educator (CNE) and Nurse Unit Manager (NUM) with after-hours access to the CNE for guidance. Your day-to-day will involve delivering direct patient careincluding assistance with feeding and showeringcoordinating admissions transfers and discharges and liaising with discharge planners nursing homes and home caregivers. Strong communication and family support will be at the heart of your work.
