Overview
We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our clients expanding team in Elizabethtown this role you will provide individualized nursing care to patients in their homes supporting recovery promoting independence and improving quality of life. The ideal candidate will have a strong clinical background exceptional communication skills and a commitment to delivering high-quality patient-centered care.

Key Responsibilities
Provide skilled nursing care and perform comprehensive assessments in home settings
Develop and implement individualized care plans in collaboration with physicians and healthcare teams
Educate patients and caregivers on treatment plans medications and post-care management
Monitor patient progress and modify care plans as necessary to ensure optimal outcomes
Maintain accurate and timely clinical documentation per agency and regulatory standards
Communicate effectively with the interdisciplinary team to coordinate seamless care delivery

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday and Sunday 8:00am 5:00pm for admissions as needed)
Two weekdays off provided following weekend on-call duty

This is a Pay Per Visit position

Travel
Local travel required within the Elizabethtown area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (assistance available if needed)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Previous home health or community-based nursing experience
Strong assessment organizational and interpersonal skills
