Overview
We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our clients expanding team in Traverse City Michigan. This role focuses on delivering skilled patient-centered care directly in patients homes supporting their recovery independence and overall quality of life. The ideal candidate will demonstrate strong clinical judgment excellent communication skills and a passion for providing personalized care.

Key Responsibilities
Conduct comprehensive patient assessments and deliver skilled nursing care in home environments
Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary care team
Educate patients and families on treatment plans medications and post-care instructions
Monitor patient progress evaluate outcomes and make necessary care adjustments
Ensure accurate and timely documentation in accordance with clinical and regulatory standards
Coordinate with healthcare professionals to support continuity and quality of care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
One weekend per month (on-call)
Two holidays per year
Flexible scheduling with minimal weekend work
This is a Pay Per Visit position

Travel
Local travel required within the Traverse City area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Michigan
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable transportation and current auto insurance

Preferred
Previous home health or community-based nursing experience
Excellent assessment communication and organizational skills
