Your Opportunity

Are you passionate about making a meaningful impact in the lives of young people As a Youth Case Manager within the Housing and Support Link Service youll be working on the front lines from the Mater Hospital Young Adult Health Centre in South Brisbane. Your role will focus on supporting young individuals who are homeless or at risk of homelessness while they receive treatment or support.

Youll collaborate closely with young people and their health and community networks to break down barriers to stable housing. Your mission is to identify and secure housing solutions that support recovery and build the skills needed for long-term tenancy success.

Key Responsibilities

  • Provide support to young people experiencing or at risk of homelessness to stabilise their housing.
  • Develop and implement individualised support and case plans including referrals to appropriate services.
  • Assist clients in transitioning out of the service and into sustainable housing.
  • Facilitate group case management sessions and group work programs.
  • Actively participate in relevant community-based meetings and networks.
  • Complete administrative tasks including internal and external reporting.
  • Document all risks incidents and safety concerns in accordance with organisational procedures.

Requirements for Success

  • Relevant degree with at least 1 year of experience or Associate Diploma with relevant experience or a combination of qualifications and substantial relevant experience.
  • Demonstrated experience working with young people with high and complex needs.
  • Sound understanding of Child Protection legislation and child safety concerns.
  • Strong crisis intervention case management referral and advocacy skills.
  • Professional approach with the ability to maintain clear boundaries.
  • Excellent stakeholder engagement and relationship management skills.
  • Strong organisational and time management abilities.
  • Proficiency in Microsoft Office and general computer literacy.

Required to get vaccination for Hepatitis B Mumps Rubella COVID 19 Measles Whooping Cough Chicken Pox and Tuberculosis as per the Biological Safety Policy.

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
