RN Care Coordinator Days (Full-Time)

Schedule

• Full-Time
• Monday-Fridayflexibleonsite during typical business hours on call between 6 am-4 pm.
• Rotating backup on-call schedule every 56 weekends along with one holiday per year.

How Youll Make an Impact

• Oversee healthcare needs of 1620 residents inourgroup homes ensuring high-quality care in a supportive home environment.
• Coordinate and attend medical appointments specialty visits and annualphysicals;complete quarterly and PRN health assessments.
• Provide phone support to direct support professionals medication managers and CMAs from 6 AM4 PM; conduct weekly (and as-needed) visits to each group home.
• Maintain medical charts in compliance with state regulations; deliver staff training in areas such as G-tube care seizure management and medication administration.

What Youll Need

• Graduateof an accredited School of Nursing and licensed to practice as a Registered Nurse in the State of will be consideredif pursing an RN degree.
• Nursing experience in the field of developmental disabilities pediatrics and behavioral health preferred.
• Musthave reliable transportation and be willing to become an authorized driver withChildServeanduseowncar to transport children in the community.
• Ability tolift upto 35lbs be mobile within the clinic areas and frequently bend lift reach and stand.
• Active BLS from the American Heart Association or American Red Cross
• Ability to understand and communicate effectively in (or through) reading writing and speaking the English language.
