Description:

The Program Coordinator is responsible for actively participating in and leading (as appropriate) the strategic planning process organizing operational processes data management and analysis and actively facilitating support of the operations and administrative components of the program. The coordinator will develop and/or support the successful accomplishment of the standards for the provision of evidence based patient care performance improvement initiatives and professional organization standards. Collaborates with physicians in the development and application of practice guidelines quality improvement mechanisms and research to enhance the quality of care to patients and their families. The Program Coordinator serves as liaison for the Program between physician specialists primary and consulting physicians clinical and support personnel and the community. The coordinator supports research education and patient care including patient advocacy and compliance with regulatory and accrediting requirements.

What the Program Coordinator will do:

  • Participates in multidisciplinary care conferences to review complex patient cases with a team approach.
  • Identifies patients at high risk for treatment failure identifying root causes and works to eliminate gaps/barriers and improve quality and access to care.
  • Collects data information for assessment of programmatic metrics of program effectiveness measures.
  • Collaborates with leadership to establish goals and metrics for monitoring the quality of the care provided as measured against industry standards. Adheres to guidelines goals and timelines set by leadership.
  • Provides education that is congruent with patient needs focusing on disease process expected side effects of treatment and community resources. Serves effectively as an educator for patient and family helping to make care seamless continuous and comprehensive promoting high quality care.

What the Program Coordinator will need:

  • Graduate of an approved nursing program. Bachelors of Science in Nursing minimum.
  • Current State of Michigan Registered Nursing License Certified Nurse in subspecialty (CVN) or within one year. BLS certified. NIHSS certification.
