Summary:
Responsible for optimizing quality data and information management to help improve patient outcomes safety operational efficiency clinical variation and value-based care. Coordinates with the local hospital Quality team to ensure the quality information structure processes and technology support effective performance improvement. Ensures the thorough review of data discrepancies analysis of data and follow-up actions to improve patient care and safety. Oversees the coordination of data and workflow to support the medical staff professional practice evaluation processes. Plans implements and evaluates performance activities to ensure adherence to internal and external quality standards.
Responsibilities:
1.Coordinates the hospital-wide quality data and information structures processes and technology that support the organizationï½s performance improvement program working collaboratively with other departments and staff.
2.Generates and analyzes a wide variety of clinical outcomes and process measure reports and dashboards to provide information for the performance improvement program through identification and analysis of both organization-wide and service line specific data utilizing appropriate data reporting tools and technology.
3.Collaborates with the hospital Quality team to create data reports and dashboards to support performance improvement activities and projects.
4.Ensures accurate and timely quality reporting analysis and communication of performance measure reports and data toall internal stakeholders from executive team to middle management to front line staff.
5.Ensures accurate and timely quality reporting analysis and communication of performance measures toall external stakeholders including Centers for Medicare & Medicaid Services (CMS) The Joint Commission state Department of Health and other quality regulatory agencies and programs.
6.Participates actively on assigned committees and work groups to support data collection analysis and performance improvement activities.
7.Coordinates the Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) processes in collaboration with Quality Director PI Specialists Medical Staff and Leadership.
8.Collaborates with other departments such as Data Analytics Information Technology Clinical Variation Supply Chain Management Value Based Care etc. as needed for success.
9.Maintain and Model Nuvance Health Values.
10.Demonstrates regular reliable and predictable attendance.
11.Performs other duties as required.
