Job Summary

Aloma Healthcare Inc. seeks a talented and experienced Care Lead to join us for a full-time role as an individual contributor. The Care Manager will be responsible for leading patient care teams and ensuring provision of quality healthcare services to all patients. The Care Lead must possess excellent communication problem-solving and customer service skills as well as the ability to remain organized and multitask.

Responsibilities
Lead patient care teams to ensure provision of quality healthcare services
Develop team performance plans goals and objectives
Oversee patient care services including scheduling tracking and evaluating
Monitor patient care processes and adjust as needed
Maintain up-to-date records of patient care services
Ensure compliance with all healthcare laws regulations and policies
Monitor patient care teams to ensure quality and compliance with safety protocols
Communicate proactively with patients management staff and other healthcare professionals to foster positive relations
Generate KPIs (key performance indicators) and PIP (performance improvement projects) and metrics to improve company base and client level improvements
Conduct and oversee quality meetings (QAPI) and conduct compliance operation audits (client and staff files)
Attend conferences and marketing events and manage media relations/external communications

80% of assignments will involve in-home care responsibilities

Requirements
Bachelors degree in related healthcare field
5 years of professional healthcare experience
Strong knowledge of healthcare regulations and laws
Excellent communication problem-solving and customer service skills
Proven ability to lead teams in a positive productive manner

Skills:

  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • Must possess excellent interpersonal skills with the ability to communicate effectively.
  • Working knowledge of office equipment preferred computer experience required.
  • Must be able to read write and comprehend English.
